Is Notion HIPAA compliant? BAA, plans and safe use

Is Notion HIPAA compliant? What Notion says about signing a BAA, which plans qualify, safe and unsafe uses for a practice, and safer options.

Is Notion HIPAA compliant? Notion will sign a business associate agreement (BAA), but only for workspaces on its Enterprise plan, after HIPAA compliance is switched on in workspace settings and Notion's recommended configuration is applied. Every other plan, including the free one, can't hold patient information. For most practices that use Notion as a team wiki or task board, the bigger risk isn't the plan. It's patient details drifting into pages that were never meant to hold them.

The question comes up often: "is notion hipaa compliant" is searched about 140 times a month (Google Ads Keyword Planner data via DataForSEO, US, 12-month average to Sep 2026).

Short answer from Notion's own documentation

Short answer (verified October 2026): Notion's HIPAA help page (opens in a new tab) says: "To be eligible to sign Notion's BAA, you must subscribe to our Enterprise Plan." You activate it under Settings > Workspace settings > HIPAA compliance > Activate, then read and accept the BAA. Notion adds that "Any Beta Services are not covered by the BAA" and that "Notion may not be used to communicate with patients, plan members, or their families or employers."

What HIPAA requires before patient information goes in

A practice may let a vendor "create, receive, maintain, or transmit protected health information on its behalf" only with written assurances that the vendor will protect it (45 CFR 164.502(e) (opens in a new tab)). That contract is the BAA, and its required terms are set out in 45 CFR 164.504(e) (opens in a new tab): permitted uses, safeguards, breach reporting, and return or destruction of the data at the end. Storing notes in a cloud workspace counts as "maintaining."

A BAA alone isn't compliance. Notion's own page lists the settings it expects under each HIPAA safeguard, and how staff use the workspace matters as much as the contract. More on contracts in business associate agreements.

Which Notion plans a BAA can cover

PlanBAA?Notes
EnterpriseYesActivated by the workspace in settings
Any other plan, including freeNoKeep all patient information out

Even on Enterprise, Notion restricts where PHI can appear. Its page says users may not put PHI in workspace or organization names, teamspace names, file names, profile fields or user group names, and support requests "must not include any PHI." Notion Mail and Notion Calendar need HIPAA set up through Google Workspace instead; see google workspace.

Notion's page doesn't say whether Notion AI, Notion Sites, forms or third-party connectors fall under the BAA. Get that answered in writing before staff use them with patient data.

Settings Notion lists for HIPAA workspaces include:

  • SAML single sign-on and SCIM user provisioning
  • Session timeouts and the audit log
  • Disabling guests (or requiring guest approval) and public page sharing
  • Disabling exports, new workspace creation and unapproved third-party extensions
  • Data retention settings

Wikis, task boards and accidental PHI

Notion gets adopted bottom-up. Someone builds an onboarding wiki, then a task board, then a "patient follow-ups" database, and nobody decides that the workspace now holds PHI. Typical drift:

  • A task card: "Call back Mrs. Lee about her biopsy results."
  • Meeting notes listing which patients complained and why.
  • A screenshot of the day's schedule pasted into a huddle page.
  • A file named "JSmith-referral.pdf," which Notion bars even under its BAA.
  • A training page using real patient examples.

Safe uses and unsafe uses for a practice

Safe without a BAAUnsafe without Enterprise and the BAA
Office policies and SOPsPatient follow-up lists or callback tasks
Staff training with made-up examplesNotes about specific patients
Marketing calendars and content draftsIntake details, referrals, insurance info
Vendor lists, job descriptions, checklistsScreenshots of the EHR or schedule

Even with Enterprise, Notion says it can't be used to communicate with patients, so it's never a patient-messaging tool.

Safer setup or alternatives

  1. Decide the boundary in writing. "No patient information in Notion" is a valid policy, and the easiest to keep.
  2. Put patient work where it belongs. Callbacks and notes go in the EHR or practice-management system you already cover with a BAA.
  3. If you need Notion for PHI, move to Enterprise, activate HIPAA, apply Notion's settings and audit pages quarterly.
  4. Compare similar tools before you switch; airtable has its own Enterprise-tier BAA rules.

Medfy doesn't sell or integrate with Notion. For its own services, Medfy signs a BAA with each practice, and patient information from website forms and chat is stored in separate, encrypted HIPAA databases in the US or Canada. Frequently asked questions

Will Notion sign a BAA?

Yes, on the Enterprise plan only. HIPAA compliance is activated in Workspace settings, where you read and accept Notion's BAA. Notion then expects specific settings, such as disabling public page sharing and unapproved integrations, and it bars using Notion to communicate with patients. Beta features aren't covered. Have your compliance officer or attorney review the BAA and your configuration before adding patient information.

Is the free version of Notion HIPAA compliant?

No. Notion says you must subscribe to its Enterprise plan to be eligible to sign its BAA, so the free plan and every other tier can't be covered. A free workspace is fine for practice wikis, policies and marketing calendars that contain no patient information. Write that boundary into your staff policy and confirm it with your compliance officer.

Can a practice use Notion without patient information?

Yes. Policies, training guides, marketing calendars, vendor lists and job descriptions don't involve PHI, so no BAA is needed. The risk is drift: a task like 'call back Mrs. Lee about her biopsy' or a pasted schedule screenshot turns a wiki into a patient record. Set a written no-PHI rule, check pages quarterly, and confirm the policy with your compliance officer.

What is a HIPAA-compliant alternative to Notion?

For patient tasks and notes, the best home is usually your EHR or practice-management system, which you already cover with a BAA. For documents, Google Workspace or Microsoft 365 with the vendor's BAA accepted can work. Notion Enterprise with HIPAA activated is also an option. Whatever you choose, configure it as the vendor specifies and have your compliance officer or attorney review it.
