AI Mode is Google's conversational search: a separate mode in Google Search where a patient types or speaks a full question, gets an AI-written answer with links, and keeps asking follow-ups. Most of the 90,500 monthly searches for "ai mode google" come from consumers curious about the feature (DataForSEO keyword difficulty 71); only about 20 a month search "google ai mode seo" (Google Ads Keyword Planner data via DataForSEO, US, 12-month average to Sep 2026). This page translates AI Mode for practices: how it differs from AI Overviews, how it handles local and health questions, and what to check on your own listings now.
In short: Google AI Mode is a conversational search experience where patients ask follow-up questions and get AI-written answers with links. Unlike AI Overviews, people choose to use it. It draws on Google's Search index and local business information, so a complete Business Profile, a page per service and recent reviews are the checks that matter.
What Google AI Mode is
Google calls AI Mode its "most powerful AI search experience" (Google Search Help (opens in a new tab)). From Google's own description:
- Ask anything, then follow up. Answers come with links to the web, and the conversation keeps context.
- Many inputs. Text, voice, images, PDFs and other files, and URLs.
- Query fan-out. AI Mode splits a question into subtopics and searches each one at the same time across multiple data sources, then combines the results.
- A fallback. If there isn't high enough confidence in an AI response, it shows a set of web links instead.
- Personalization. For signed-in users 18 and older with Search history on, it can tailor responses using previous searches.

AI Mode vs AI Overviews
Both are Google features built on the same Search systems, but they behave differently:
| Item | AI Overviews | AI Mode |
|---|---|---|
| Where it appears | Above regular results on a normal search | A separate mode the person opens |
| When | Only when Google's systems judge it helpful | Whenever the person chooses it |
| Conversation | A single summary; on mobile, people can continue into AI Mode | Follow-up questions with context |
| Links shown | Supporting links beside the summary | Links in the answer; web links instead when confidence is low |
| Sources | Google says the two "may use different models and techniques", so the links can differ (Google Search Central (opens in a new tab)) | Same caveat |
For a practice, the difference that matters is intent. A patient in AI Mode is often in a longer, decision-making conversation: comparing options, filtering by insurance or hours, asking what to expect. That's closer to choosing a practice than a one-off question is. Our hub on Google AI Overviews covers the summaries in detail.
How AI Mode handles local searches
Google has described local results in AI Mode in its announcements:
- Place cards. In May 2025 Google said that for local spots, "like restaurants, salons and stores, you can quickly see info like ratings, reviews, and opening hours," with the option to call or get directions from the answer (Google (opens in a new tab)). Medical offices weren't named in the examples, so check how your own specialty appears.
- Business information in answers. Google's generative AI guide says responses can include local business information and that Business Profiles can help businesses be visible in AI responses (Google Search Central (opens in a new tab)).
- Booking ambitions. Google said in May 2025 that agentic features, where AI Mode helps complete tasks, would "start with event tickets, restaurant reservations and local appointments." Treat that as a direction of travel, not something to plan around for patient booking.
The local ranking rules Google publishes still apply: results are based mainly on relevance, distance and how well-known a business is, and "there's no way to request or pay for a better local ranking on Google" (Google Business Profile Help (opens in a new tab)). For how other assistants handle "near me" questions, see local AI search.
Health questions in AI Mode
Google's AI Mode help page doesn't single out health. It says responses are grounded in high-quality web content, and it also warns that AI Mode can misread web content or miss context. Two features of the design matter for health:
- The confidence fallback. When Google isn't confident in an AI answer, it shows links rather than a summary.
- The same quality systems. Google says its generative AI features are rooted in its core Search ranking and quality systems, the same ones that rank health pages in regular results.
What that means for a practice: you won't control how AI Mode summarizes a condition, and national health sources will often be the ones linked for "what is" questions. Where you can help is the practical layer. If a patient asks "which dermatologists near me do Mohs surgery and take Medicare?", your best chance is a page that says so in plain words, and a profile that agrees with it.
What feeds AI Mode: your site, your profile, your reviews
Google doesn't publish AI Mode ranking factors. What it does document:
- Your website, through the Search index. Google's generative AI features retrieve indexed pages and show supporting links. A page must be indexed and eligible for a snippet; Google says there are no extra technical requirements and no special markup.
- Your Business Profile. Google recommends keeping Business Profile information current as part of its guidance for AI features.
- Your reviews and ratings, which Google shows on place cards for local spots.
Anything beyond that, including claims about secret AI Mode signals, isn't supported by Google's documentation.
What practices should check now
- Business Profile: correct primary category, every service listed, accurate hours including holidays, relevant attributes, and current photos. Our guide to Business Profile optimization walks through each field.
- A page per service on your website, stating who it's for, insurance accepted, new-patient availability and the clinicians who provide it.
- Recent reviews, requested steadily, with replies that never confirm someone is a patient.
- Search Console. Since June 2025, AI Mode data counts toward Performance report totals (Google Search Central (opens in a new tab)), and the generative AI performance report (opens in a new tab) shows impressions when your links appear in AI Overviews and AI Mode, by page.
- Try it yourself. Ask AI Mode the questions your patients ask, then two follow-ups ("which take Aetna?", "which are open Saturday?"). Note whether you stay in the answer.
- Crawl access. Make sure Googlebot isn't blocked from your service pages.
Medfy's weekly AI visibility tracking covers Google AI Overviews, plus ChatGPT, Gemini, Claude and Perplexity; it doesn't check AI Mode, so step 5 stays a manual check. No one can promise a mention; answers vary by phrasing, location and day.
Frequently asked questions
What is Google AI Mode?
AI Mode is a conversational mode in Google Search. A person asks a full question, by text, voice or image, and gets an AI-written answer with links to the web, then asks follow-ups. Google says it splits questions into subtopics and searches them in parallel, and shows ordinary web links when it isn't confident in an AI answer. Google calls it its most powerful AI search experience.
Is AI Mode available in Canada?
Yes. Google began rolling out AI Mode in Canada on August 21, 2025, in English first, as a tab on the Search results page and in the Google app. As of October 2026, Google's AI Mode help page lists Canada among supported countries and French among supported languages. Features can vary by country and account, so check what patients in your area see.
Does AI Mode show local businesses?
Yes. Google has said AI Mode shows place cards for local spots with ratings, reviews and opening hours, and lets people call or get directions. Google's guidance also says Business Profiles can help businesses appear in AI responses. For a practice, that makes a complete, accurate Business Profile and recent reviews the most practical things to check.
How is AI Mode different from AI Overviews?
AI Overviews appear automatically above regular results when Google judges a summary helpful. AI Mode is a separate experience people choose, built for follow-up questions and comparisons. Google says the two may use different models and techniques, so the links they show can differ. Both draw on Google's Search index, and both are counted in Search Console.
